I, the parent or legal guardian of [Child’s Name], hereby acknowledge and agree to the following terms and conditions for my child’s participation in class at Crooked Tree Arts Center:
1. I acknowledge that there may be certain inherent risks associated with art activities, including but not limited to slips or falls.
2. I acknowledge that it is my responsibility to provide any necessary and helpful information about allergies or other conditions that may impact my child’s participation.
4. I understand that Crooked Tree Arts Center, its staff, volunteers, and affiliates will take reasonable precautions to ensure the safety and well-being of all participants. However, I acknowledge that accidents or unforeseen events may still occur despite their best efforts.
5. In consideration of my child being allowed to participate in art class activities, I hereby release, waive, discharge, and hold harmless Crooked Tree Arts Center, its directors, officers, employees, and volunteers from any and all claims, liabilities, or damages arising from or in connection with my child’s participation in the class
6. I agree to indemnify and hold Crooked Tree Arts Center harmless from any claims or actions brought against them as a result of any act or omission on the part of my child during the camp activities. I have read this waiver and fully understand its contents. I voluntarily agree to the terms and conditions stated herein and acknowledge that it is binding upon me, my child, and our heirs, executors, and administrators.
